When placing an order for a patient at one of the hospitals please supply us with a home address. You may do this in the special instructions area of the order form. If you do not know the home address please verify they are still located at the Hospital. At times when attempting delivery we find they are no longer in the hospital and we cannot obtain an address from the Hospital do to their confidentiality policy.
